DSPC may require all employees, OJTs, customers, suppliers, contractors and visitors to submit a Health Questionnaire and competent proof of identification and/or to submit to contact tracing procedures prior to entering company premises should there be a public health emergency.

Rest assured that all personal information presented shall be handled with the highest degree of confidentiality and retained only during the duration of the public health emergency, in the secure database of DSPC, while accomplished Health Questionnaires shall be securely disposed of after thirty (30) days from execution.
